If your child needs to be referred to a specialist, please call us at (623) 877-7337. Our dedicated referral coordinators can help you find the specialist you need, answer many questions having to do with insurance, and generally help make your referral as smooth as possible.

Depending on your insurance, many referrals require an office visit with your primary care provider. Please call your insurance company at the number listed on your insurance card to learn about the policies of your plan.

Requesting Medical Records

To have medical records sent to another provider, please fill out the medical records release form and fax it back to your primary office location.
